Requote Checklist - Auto

Help us ensure the most accurate and competitive requote for your auto insurance. We already have your current policy, and this form with help us be the most up to date with any changes that may affect coverage or pricing.

  • What is the best contact phone number that we should have in your file?
  • What's the best email address that we should have in your file?
  • When our team needs to contact you
  • If you've changed your mailing address or physical address, please add the new address here.
  • Are any of your vehicles garaged at a different location, other than your home? If so, please explain. *This may require additional follow up.
  • Household Driver Updates

  • Has there been any change in your household drivers? If so, please describe the changes and person's name. Examples: 1) Household member has moved out. 2) New household member has moved in. 3) Is there anyone else that is regularly driving your car that's not listed on the insurance?
  • Has any driver received a ticket or been in an accident? If so, please include the date, description and who was involved.
  • Does any driver qualify for either of these discounts: 1) Defensive Driving Course 2) Student: Good Student Discount (GPA 3.0+)
  • Are you a member of AAA Roadside Service?
  • Has any driver on your policy had their licenses suspended or reinstated? Or needing an SR22.
  • Household Vehicle Updates

  • Have there been any changes in the vehicles listed on your policy? Examples: Vehicles sold, purchased, in storage, loaned out, customized or altered, refinanced or a loan paid off
  • Do you often drive vehicles owned by others, including your employer's vehicle? Or use your vehicle(s) for business reasons? Do you use your vehicles for ride-share Lyft, Uber, Turo or other?
  • Do you have any custom equipment on your vehicle(s)? Examples: Stereo systems, aftermarket parts, grill guards, etc. If so, please describe and give an estimated value of the customization.
  • Please let us know the estimated annual miles driven by your vehicle(s).
  • Coverage & Quote Preferences

  • If you would like to see any changes to your coverages, please describe. This can include 1) Deductible changes. 2) Adjusting Comprehensive & Collision (may be heard as "Full Coverage" - on any of the vehicles). 3) Adding or Removing Roadside Assistance or Rental Car Coverage. If not, we will quote using your current coverages.
